Abstract: The Application Form refferred to in Article IX of the Rules of Procedure of the Administratuve Tribunal shall consist of questions under the following: - Information concerning the personal status of the Applicant. - Official status of Applicant. - The challenged decision. - Channels of administrative review of the decision that applicant has persued and the result. - The pleadings (relief claimed, list of documents).
